"I am out of my medication and need a refill"

To request a prescription renewal, please ask your pharmacy to fax us at: 

226-314-3303

Please note that we will not renew prescriptions for patients who have not completed any follow-up as discussed at your first appointment.

"My lab requisition expired and the lab requires a new one"

Please email us, and we can send you a new one via the patient portal

"I have not received an email as discussed"

Please check your junk/spam folder! Mark the clinic as non-spam, we have noticed that sometimes mail systems will automatically send it there.

"What is the clinic address?"

We will provide you with the clinic address when you are a registered patient, as we take safety and discretion into consideration for everyone.

"I have a virtual appointment, and the doctor is not here?"

Similar to the clinic setting, previous appointments can go over allotted time based on needs. Please remain available, as our doctors will call you if you are not online.
