Gender Affirm Guelph is gender affirming clinic that first opened in 2020 and later amalgamated with the ARCH clinic. We are now returning to our original model of care where we can provide compassionate OHIP-covered gender affirming care to our community as well as those living in other parts of Ontario. Our care is offered by skilled doctors and nurses both virtually and in person. There are no fees for services - it is fully covered by OHIP. We see patients aged 12 and up.
At Gender Affirm Guelph, our mission is to provide quality healthcare to all of our patients. We strive to create a welcoming and inclusive environment where everyone can feel comfortable seeking medical care.

Please DO NOT use this link if you are a new patient who has not yet been seen by Dr. Jiwa. For information on how to become a patient, please see below for information on our referral process.
New patients can either self-refer or have a referral faxed over by their family doctor. Patients who live far from Guelph who want to be seen only virtually will need a doctor's referral faxed over. Patients who can attend occasional in person appointments can self-refer.
